Library Cards

Get your Fitchburg Public Library card by completing a card application and visiting the library in person. Please bring a photo I.D. and proof of residence.

If you are unable to come into the library, please fill out our online library card application form, and a member of our staff will respond to you as soon as possible.

Identification Options

A driver's license with current residence is the easiest way to apply. If you do not have that, we will accept photo ID plus:
  • Checkbook with name and current address
  • Current lease with name and address
  • Piece of mail if the envelope has your name, current address, and a postmark dated within the last 30 days
If your identification has a P.O. box or business address, you must still show proof of your current home address.

Anyone under 16 years of age must have a parent or guardian present in order to get a library card.

Lost Cards

Please report lost or stolen library cards immediately by calling (608) 729-1761. Any items checked out are the responsibility of the card holder. You can obtain a replacement card at no cost by visiting the library's first floor Information Desk.
